THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our duties
Project Wellbeing is required by law to maintain the privacy of your Protected Health Information ("PHI"), provide you this notice of our legal duties and privacy practices, and abide by the terms of the notice currently in effect.
Permitted uses and disclosures
- Treatment — to coordinate care among our therapists, trainers, and outside providers you authorize.
- Payment — to bill you, process cards, and manage memberships.
- Healthcare operations — for quality improvement, training, and administrative functions.
- As required by law — including public health, abuse reporting, and judicial orders.
Uses requiring your authorization
Most uses and disclosures of psychotherapy notes, marketing communications, and any sale of PHI require your written authorization, which you may revoke at any time.
Your rights
- Right to inspect and copy your PHI.
- Right to request amendments to inaccurate records.
- Right to an accounting of certain disclosures.
- Right to request restrictions and confidential communications.
- Right to a paper copy of this notice on request.
- Right to be notified of a breach of unsecured PHI affecting your information.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at info@projectwellbeing.co or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
